Symptom

When you activate a table with indexes, errors occur because indexes with identical columns exist. However, one index is an extension index that is not active, but which is defined identically in a new version.

When you activate a table, the system also checks the latest version of the extension indexes, especially those in the new status, which may not be activated at all in this system.
Therefore, the system reports an error, for example, if a table has an index whose defined columns correspond with the columns of the new extension index.
Solution

If the table was not changed, you can work at index level and activate explicitly there. There, the system checks only active indexes, and not inactive versions of an extension index in particular.
You may be upgrading or you are importing enhancement packages. In this case, a changed SAP-specific table must always be active, since this table may be used for follow-on actions. If there is an identically defined customer index in addition to the extension index, you can delete it. After you upgrade or import the packages, you can implement the correction instructions and create the customer index again.
As a result, you are not required to implement the note during the activation (possibly in the shadow system).
After you implement the attached correction instructions or inport the relevant Support Packages, the error no longer occurs when you activate the table.
